Position Overview
The Community Outreach Specialist will establish ClinNEXUS's first presence in the Monterey / Santa Cruz / Salinas region, identifying Medi-Cal members eligible for Enhanced Care Management (ECM) and building the referral relationships needed to grow the program from the ground up. The role blends community outreach, partner development, member engagement, and enrollment support, with significant time in the field at health fairs, FQHCs, shelters, clinics, and behavioral health settings. Success here means converting local relationships into a sustainable referral pipeline.
Key Responsibilities
- Establish ClinNEXUS's presence in the region through health fairs, community events, churches, shelters, food distribution sites, and behavioral health settings.
- Build referral partnerships with CBOs, FQHCs, hospitals, clinics, behavioral health providers, social service agencies, housing organizations, and county programs.
- Educate community partners on ECM eligibility, member benefits, referral pathways, and the ClinNEXUS care model.
- Identify and engage potential ECM members in person, by phone, or in the field; explain services and address barriers to participation.
- Conduct preliminary eligibility screening against ECM criteria, obtain member consent, and convert qualified referrals into active enrollments, in coordination with intake, authorization, and care management teams.
- When assigned, serve as the onsite ECM representative at a partner site or discharge setting, working with discharge planners and care transition teams to identify ECM-eligible patients. Provide feedback to leadership on workflow barriers and ways to strengthen referral and discharge-management processes.
- Document outreach, referral, screening, and enrollment activity, and maintain organized pipeline tracking with status and outcomes.
- Track and report key metrics (referral volume, conversion, partner engagement, event activity); participate in market development and care team meetings.
- Conduct or support initial SDOH screening (e.g., PRAPARE), escalate urgent member needs, ensure a warm handoff to the care team post-enrollment, and support early member engagement during onboarding to reduce drop-off.
- Maintain confidentiality and comply with DHCS, payer, HIPAA, consent, and documentation requirements.
Qualifications
- 2+ years of experience in healthcare outreach, community health work, care coordination, intake, enrollment, or social services.
- Strong understanding of Medi-Cal/Medicaid programs, including CalAIM, underserved populations, and complex care environments.
- Experience managing referral pipelines with accurate, timely documentation, and engaging members across phone, in-person, and field-based settings.
- Ability to work independently in a field-based role while meeting team goals and compliance requirements.
- Strong relationship-building skills with community partners, providers, and diverse populations, with the ability to influence stakeholders effectively.
- Demonstrated problem-solving ability and comfort operating in evolving or developing environments.
- Ability to translate community insights into actionable strategies that drive growth.
- Highly organized, with effective, mission-aligned communication and documentation habits.
- Proficiency with Google Workspace, HealthCloud for Salesforce, or similar platforms.
- Valid CA driver's license, reliable transportation, proof of insurance, and a clean driving record.
- Able to meet credentialing requirements (background check, drug test, FACIS, licensure verification).
- Preferred: Associate's/Bachelor's degree; direct ECM/CalAIM/Medi-Cal managed care experience in this region; background as a Community Health Worker, Promoter, Medical Assistant, or Case Manager; existing relationships with healthcare or community organizations in Monterey / Santa Cruz / Salinas; Spanish fluency, strongly preferred given the region's agricultural communities.
Work Environment
- Field-based role centered in the Monterey / Santa Cruz / Salinas region, with a remote home base and reliable internet access.
- Preference given to candidates living in or with established ties to the area; candidates originally from the region willing to travel back will also be considered. Requires frequent local travel and the physical capacity for extended standing, walking, and community-based work.
Compensation
- Pay Range: The good faith pay range for this position is $72,000 to $90,000 annually, plus bonus eligibility. The specific pay rate within this range will be based on the candidate's qualifications, including experience, skills, and geographic location.
