As a Direct Support Professional for the Day Habilitation program is responsible for assistance and support to participants’ individual goals, recreational activities, and community involvement while maintaining the individual’s personal comfort. Day Hab activities take place on Monday's from 1pm to 5pm, Tuesday from 4:30pm to 8:30pm, and Friday from 8am to 1pm.
Key Responsibilities

Interacts with the clients to support the completion of daily goals as described in each client’s Individual Service Plan (ISP).
- Provides the necessary support to allow individuals to participate in recreational activities and facilitate community involvement.
- Assist in the administration of medications that are ordinarily self-administered according to company policy.
- Monitors and reports any observed changes in the client's condition and/or needs to the supervisor.
- Documents the care provided by entering the required information into the client's records.
- Promotes personal safety and maintains a safe environment for clients by observing infection control practices.

Effectively communicates with all members of the interdisciplinary team through verbal reports and staff meetings.

Maintains confidentiality in all aspects of the position.
- Attend in-service programs and trainings to meet compliance requirements.

Performs other related duties and responsibilities as deemed necessary.
Position Requirements

At least 18 years old

At a minimum, a high school diploma or GED

A passion for supporting others, respect for individual differences, and the ability to advocate for the best plan of action for the individuals we serve are the most important requirements of this position.

Sound judgment and good decision-making skills are extremely important.

A willingness to adapt current skills and learn new skills and new ways to deliver services to the individuals we serve is crucial to personal and professional growth.

An acceptable driving record is preferred to assist individuals in the community.
