The Pisgah Institute is a private behavioral health practice dedicated to providing compassionate, high-quality mental health care. We're seeking an organized, detail-oriented Patient Referral Coordinator to serve as a critical link between patients, referring providers, and our clinical team.
About the Role
The Patient Referral Coordinator manages the full referral lifecycle, from initial intake through scheduling, ensuring patients connect smoothly with the right level of care. This role requires strong communication skills, comfort navigating communication and EHR systems, and a genuine commitment to supporting patients during a vulnerable time in their care journey.
Key Responsibilities
- Receive, process, and track incoming referrals from physicians, hospitals, and other community partners
- Conduct initial outreach to referred patients and gather necessary intake information
- Match patients with appropriate clinicians based on clinical needs, availability, and insurance coverage
- Maintain accurate, up-to-date referral and patient records in the EHR system while ensuring HIPAA compliance
- Communicate proactively with referring providers regarding referral status and any barriers to care
- Coordinate with front desk and clinical staff to schedule intake appointments efficiently
- Follow up with patients who have not responded to reduce drop-off and improve access to care
- Identify and troubleshoot barriers to treatment access, such as insurance issues or scheduling conflicts
· Assist clinical staff with outgoing referrals, including coordinating follow-up information and supporting continuity of care
Qualifications
- High school diploma required; associate or bachelor’s degree preferred
- 1+ years of experience in a medical office, behavioral health, or referral coordination role
- Familiarity with EHR systems and insurance verification processes
- Strong interpersonal and phone communication skills, with a patient-centered approach
- Excellent organizational skills and ability to manage multiple referrals simultaneously
- Understanding of HIPAA and patient confidentiality requirements
- Knowledge of behavioral health terminology and levels of care is a plus

Pay: $17.00 - $18.00 per hour
