Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts or transfer belts
- Support patients with complex needs such as dementia, Alzheimer's disease, or developmental disabilities through specialized memory care techniques
- Follow guidelines and HIPAA regulations to ensure patient safety
- Help with meal preparation, feeding, laundry, light cleaning, and maintaining a safe home environment to promote comfort and hygiene
- Maintain detailed medical records and documentation of patient interactions in compliance with healthcare standards and privacy laws
Skills
- Strong patient observation skills to detect changes in health status promptly
- Experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience
- Knowledge of medical terminology relevant to home health care settings
- Ability to assist with ADLs efficiently while maintaining dignity and respect for clients’ independence
- Experience caring for individuals with dementia, Alzheimer's disease, or other cognitive impairments including memory care strategies
- CPR certification and familiarity with hospice care practices are highly desirable
- Competence in vital signs measurement, medication administration, catheter care, and hospice support tasks
- Ability to work independently or as part of a team providing home & community care services
- Flexibility for scheduled visits tailored to client needs
