Position Summary
Mountain View Health & Rehabilitation is looking for a compassionate, motivated, and team-oriented Physical Therapist to join our growing rehabilitation team.
Our therapy department is a cornerstone of the care we provide. We believe therapy should be more than completing exercises—it should help residents regain confidence, maintain independence, and get back to doing the things that matter most to them.
The ideal candidate is someone who brings clinical expertise, positive energy, creativity, and genuine compassion to their work. You will have the opportunity to work closely with residents, families, nursing, therapy, and other members of the interdisciplinary team to provide individualized, resident-centered care.
Essential Duties & Responsibilities
- Evaluate residents to determine physical therapy needs and develop individualized treatment plans.
- Provide skilled physical therapy services in accordance with physician orders, care plans, and applicable regulations.
- Develop treatment goals based on each resident's medical condition, functional abilities, and personal goals.
- Provide therapeutic exercises, functional mobility training, gait training, balance activities, strengthening, and other appropriate interventions.
- Assist residents with improving mobility, strength, coordination, balance, endurance, and overall functional independence.
- Monitor residents' progress and modify treatment plans as appropriate.
- Document evaluations, treatments, progress, and discharge information accurately and timely.
- Communicate changes in residents' condition or functional status to the appropriate members of the care team.
- Collaborate with nursing, occupational therapy, speech therapy, social services, dietary, and other disciplines to provide comprehensive care.
- Participate in care plan meetings and interdisciplinary team discussions as appropriate.
- Educate residents and families regarding treatment plans, safety, mobility, exercises, and strategies for continued progress.
- Recommend and assist with appropriate adaptive equipment, positioning, mobility devices, and safety interventions.
- Promote fall prevention and safe mobility throughout the facility.
- Assist with discharge planning and provide recommendations for continued therapy or services when appropriate.
- Maintain compliance with facility policies, professional standards, and applicable federal and state regulations.
- Maintain a clean, safe, and organized therapy environment.
- Participate in quality improvement initiatives and facility programs as requested.
- Perform other duties as assigned.
What We're Looking For
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in the State of Montana or eligibility for Montana licensure.
- Current CPR certification or ability to obtain as required.
- Strong clinical assessment and treatment skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of an interdisciplinary team.
- Strong organizational skills and attention to detail.
- Compassionate and respectful approach to working with older adults and individuals with varying levels of ability.
- Ability to motivate residents while maintaining appropriate clinical boundaries.
- A positive, flexible, and solution-oriented attitude.
