PRIMARY OBJECTIVE OF POSITION:
The primary purpose of this position is to ensure the safety and well-being of the individuals LEGACY ENDEAVORS, LLC provides services to, while managing and supporting their team of Personal Support Advocates, implementing individual outcomes, documenting progress, and response to program services. The Program Coordinatorwill carry out the principles and values of connecting individuals with family, friends, and community members, in meaningful ways, in shared spaces, and through ordinary life events. This will include guiding the individual and community member to discover common interests and promoting that people with disabilities can and do have a valued social roles. The Program Coordinator will role model and lead by example while co-planning, encouraging, and participating in daily life choices that reflect good physical, emotional, social, and spiritual health for the people they support.
MAJOR AREAS OF ACCOUNTABILITY:
- Obtains and demonstrates a working knowledge of individual(s) history, preferences, skills, and areas of vulnerability, individual outcomes, and Emergency Procedures. Obtains and demonstrates a working knowledge of program policies and procedures. Provides support, supervision, and safety to each person, using therapeutic interventions as specified within procedures, and directed by the person’s CSSP Addendum, Self-ManagementAssessment (SMA), Individual Abuse Prevention Plan (IAPP) Program Abuse Prevention Plan (PAPP), and other support documents including the County CSP and/or CSSP. Ensures proper distribution and documentation of all prescribed medications, treatments, and diets. Facilitates and fosters natural connections between people supported and community members Assists in increasing the individual’s comfort in engaging community members Utilizes provided tools when assisting individuals in making connections in the community Orders, maintains and performs proper distribution and documentation of all prescribed medications, treatments, and diets as directed. Assists individuals learning or completing daily living skills, and/or provide personal care as needed. Assists individuals in accessing their neighborhood or community by teaching them how to use public transportation or by transporting them in a company or personal vehicle. Demonstrates and maintains therapeutic relationshipsand uses person-centered approaches with all individuals LEGACY ENDEAVORS serves. Demonstrates and maintains positive and professional working relationships and open communication with LEGACY ENDEAVORS staff and various other persons related to the position. Encourages and provides opportunities for individual’s choice, self-determination, and presenting creative options. Abides by MN Statutes, Chapter 245D, Positive Supports Rule 9544, Human Services Licensing Act, Maltreatment of Vulnerable Adults Act and the Maltreatment of Minors Act. Ensures the accurate, legible, and objective recording and reporting of all required documentation, including, but not limited to: Daily Log Notes/Case Notes, Health Progress Notes, Outcomes and Data Collection, Medication/Treatment Records, Incident and Emergency Reports, and Medical Referral Form. Adheres to LEGACY ENDEAVORS Code of Conduct/Work Rules. Completes all required training as assigned by the company, both initially and on-going, per above mentioned Rules and guidelines, within the designated time frames. Attends staff meetings as assigned by Supervisor. Provides the following information regarding individuals served to authorized callers per the direction of the Supervisor: current medications, current physician, dentist, psychologist, psychiatrist, etc., IAPP, PAPP, behavior outcomes, SMA, and CSSP-Addendum, guardian telephone number, Medical Assistance or Insurance number, and current staff schedule. Tracks and schedules medical and dental appointments Facilitates individual’s participation with life events, family members, and in their community, including, but not limited to: birthdays, family holidays, telephone calls, sporting events, volunteering, worship services, etc. Assures general household tasks are completed as instructed by supervisor, including, but not limited to: cleaning, laundry, watering plants, snow and ice removal, reporting items that need repair, ensuring there are adequate and appropriate supplies and food, etc. Keeps staff, supervisory staff, and any other personnel inside or outside the agency informed of incidents, accidents, emergencies, and significant events and/or activities. Assures that the living and working environment is safe, harassment and discrimination free. Ensures all issues, concerns, and repairs/maintenance needs are identified, communicated, and followed up on. Reports to Supervisor immediately of any missed deadlines, household or program needs, non-performance of job duties, missing monies or items, or any other issue. Obtains and maintains necessary and required knowledge, information and skills to successfully and accurately perform the various job duties and tasks as assigned, including but not limited to:a. Daily monitoring of phone messages b. Verbal and/or written communication of phone messages to Supervisor as necessary in a timely mannerc. Daily monitoring of medication treatment records to verify medications/treatments were givend. Daily monitoring of stock of medical suppliese. Daily monitoring of functionality of medical equipmentf. Re-ordering medication prior to medication being depletedg. Re-stocking medical supplies prior to depletionh. Reporting all malfunctioning medical equipment to supervisor, RN or emergency cell phone carrier as soon as noticedi. Monthly put out accurate, up-to-date medication/treatment recordsj. Update medication/treatment records if/when medication changes occurk. Plan activities per individual interest, choice and outcomesl. Write schedule of activities on calendarm. Ensure directions are available to activities from the homen. Monitors groceries and supplies and initiates and performs grocery and supply shopping within dietary restrictions, if any, and selections that promote healthy eating, and within household budgeto. Completes 245D review and recommendations for IDT meetingsp. Monitors the documentation of house petty case and persons’ served spending money
- Ensures staff have been trained how to meet the Coordinated Service and Support Plan (CSSP) expectations and implement the CSSP Addendum, and document on outcomes. Maintains a current understanding of state licensing requirements and company specific compliance and translate to operational procedures. Provides support, training, and mentoring to PSA staff. Creates, directs, and directly supervises the work of the PSA staff. Provides feedback, guidance, and corrective measures, in coordination with the Program Director, to PSAs regarding their work performance Reports any perceived, suspected, or known dissatisfaction or potential for dissatisfaction or internal persons served, staff, or external customers. Participates in the hiring, training and on-going training of staff to adhere to licensing requirements. Plans and maintains house schedule while staying within the budget. Approves hours for payroll and ensures staff are using the clock in system correctly. Carries an on-call phone ensuring required and adequate staffing ratios according to licensing requirements are met. Participates as a team member and offers suggestions, voice concerns and provides information regarding client’s program. Ensures an employee culture that is in the best interest of the persons served, department and the organization. Assures the execution of admissions, demission, closings, transfers, and program planning processes. Contributes to the development of and assures the administration of all policy, procedure, and record keeping systems related to service operations. Actively looks for, promotes, plans and participates in opportunities for people to be involved in the most integrated opportunities. Other duties as assigned.
WORKING CONDITIONS:
- Is subject to frequent interruptions, imposed deadlines and frequent problem solving activities. May be subject to hostile and/or emotionally upset individuals receiving services, community members, family members, visitors and personnel from other agencies. May be exposed to a variety of conditions, infectious diseases, odors, dust, etc. throughout the working hours. Regularly travels in the community. May drive company vehicle and/or personal vehicle transporting people. Is subject to participating in individual’s activities and likes. Is subject to flexible hours, including split shifts or varied days and times including mornings, afternoons, evenings, weekends, and overnights, depending on coverage needs. Is able to work independently without direct supervision. Work locations and assigned individuals may vary according to the needs of the company and staffing requirements, which may include inside the program sites and/or various community locations. May be assigned to work at any of the Legacy sites, including reassignment to a different shift when the scheduled shift has been cancelled. May support people who are upset and express their feelings in physically aggressive way. Must provide positive support strategies to help the person and may need to use a emergency use of a manual restraint as the last resort. May be exposed to animals, including but not limited to dogs, cats, pet rodents, birds or pet reptiles. Is subject to a dress code which is appropriate to the assignment.
PHYSICAL DEMANDS:
- Sits, stands, walks, bends and moves intermittently during working hours. Ability to lift and transfer individuals as needed. Ability to lift at a minimum of 50lbs. Ability to master the use of all physical intervention and assistance techniques, including exempt and controlled procedures, necessary to meet the needs of assigned people. Must possess sight/hearing senses or use prosthetic services that will enable these senses to function adequately. Possess good health and demonstrate emotional stability. May need to participate along with an individual in exercise DVD’s; walking club, bike riding, fitness classes, etc.
KNOWLEDGE AND SKILL REQUIRED:
- Must be able to read, write, and speak the primary language of the individuals LEGACY ENDEAVORS, LLC provides services to. This must include the English language, and may include additional languages as needed. Meet licensing requirements of the state and/or county. High school diploma or GED required. Possess a valid driver’s license and acceptable driving record. Possess personal vehicle with the ability to transport at a minimum three additional people. Must be able to demonstrate the ability and results of building and maintaining relationships between people receiving services and community members.
Pay: $23.00 - $25.00 per hour
