MOW Specialist Position Description
Assist the Meals on Wheels Coordinator with the following activities:
- Outreach to peers to do intake for short term meal delivery.
- Run three audits per month and notify meals are ending.
- Process long term applications for Program Coordinator to review.
- Adhere to grant requirements, including deadlines, activities, and reports
- Respond timely in a peer-centered manner to telephone calls and emails.
- Organize and maintain required records in the database and excel log.
- Identifies system issues and communicates them to the MOW Coordinator
- Participate as an active team member to further the mission of the VCIL
- Participate in required trainings
- Work in a manner consistent with VCIL's philosophy of peer empowerment, self-advocacy and peer support
- Document activities for management, advocacy and development purposes. Support other VCIL programs and staff
- Other Duties as assigned.

SWFF Coordinator Position Description
Assist peers with obtaining assistive devices and services that will help them live independently in their own homes and gain control of their lives through the Sue Williams Freedom Fund (SWFF) assistance program.
Essential Functions:
· Assist with Sue Williams Freedom Fund Program projects and grant services, including but not limited to:
· Assist peers in developing personal goals for independent living
· Assist peers one on one to access small grant and loan funds for equipment, and other related supports and services to achieve personal goals for independent living
· Refer peers to other programs and funding resources, as appropriate
· Network with human service providers and others
· Maintain good working relationships with vendors
· Participate in outreach, community meetings and advocacy activities
· Assist with identifying system issues
· Respond in a timely and peer-centered manner to telephone, written and in-person requests for information
· Provide assistance in a manner consistent with VCIL’s philosophy of peer empowerment, self-help, and peer support
· Participate in training and job development activities
· Networks with human service providers and others
· Attends and participates in regional team and home & community access advocacy meetings and other meetings as directed
· Maintain the VCIL database, waiting and working lists
· Responsible for quality outcome
· Other duties as assigned
