Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or wheelchairs
- Monitor and document patient observations including vital signs, medication administration, and changes in condition using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Support clients with medication management, ensuring proper administration according to medical instructions while maintaining HIPAA compliance
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions through tailored memory care techniques
- Assist with medical procedures such as catheter care, wound care, or hospice support as directed by healthcare professionals
- Prepare nutritious meals and assist with meal planning while considering dietary restrictions and medical needs
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall well-being
- Document all care activities accurately in medical records and communicate effectively with healthcare teams to ensure continuity of care
Qualifications
- Proven experience in home health, assisted living, nursing home settings or long-term care environments
- Knowledge of medical terminology and familiarity with EMR/EHR systems for documentation purposes
- Certifications such as CPR (Cardiopulmonary Resuscitation), first aid, and training in dementia or Alzheimer's care are highly desirable
- Ability to work independently while following detailed care plans and protocols
- Strong observational skills for patient monitoring and recognizing changes in health status
- Compassionate attitude with excellent communication skills to build trust with clients and their families
- Physical ability to assist with mobility tasks and perform caregiving duties safely
