We are looking for full-time or part-time LPN's or Certified Home Health Aid's for our Overnight (10p-6a) shift for our Assisted Living Facility. Traditional and Special Care Units, must have reliable transportation, attention to detail, and a caring attitude.
Responsibilities and Duties:
- Charge person in the absence of the Resident Care Director, Assistant Administrator, and Administrator.
- Will assist and supervise medications and treatments to Residents as directed by the Resident Care Director and as ordered by the Resident’s physicians
- Records, observes and reports data concerning medications and treatments
- Documents pertinent information in the Resident Care Progress Notes for Case Management
- Acts as liaison with residents, physicians, families, friends and/or responsible parties
- Responsible for all aspects of resident care and safety
- Assist in the planning and implementing of care plans
- Assist in Quality Assurance
- Will be expected to develop and maintain a positive public relations image in the community
- Will be in compliance with local codes, NYS-DOH and Controlled Substances regulations
- Supervises Resident Care Assistants and Universal Workers in providing Resident care
- Gives change of shift report with necessary information, problem identification and provides continuity of nursing care
