About Us
Summit ReEntry is a structured Intensive Residential Treatment Services (IRTS) program dedicated to supporting individuals experiencing mental health challenges. Our mission is to provide compassionate, client-centered care that promotes stability, recovery, and reintegration into the community.
We are currently seeking Behavioral Health Practitioner for all shifts to provide direct support services to clients, including skills training, crisis intervention, treatment planning, and group facilitation.
Responsibilities:
- Conduct intake assessments and complete required documentation in the electronic health record (EHR).
- Provide individual and group skills training to improve independent living, emotional regulation, and social skills.
- Assist with treatment planning and ongoing updates to meet client needs.
- Offer crisis assistance and intervention when needed.
- Support clients in daily living activities and community reintegration.
- Ensure compliance with client rights, HIPAA regulations, and 245I standards.
- Document all client interactions and services in a timely and accurate manner.
- Work collaboratively with a multi-disciplinary team, including therapists, peer specialists, and case managers.
- Facilitate structured therapeutic and educational groups aligned with treatment goals.
- Provide medication administration support as trained and approved.
Shifts Available:
- 1st Shift: 7:00 AM – 3:30 PM
- 2nd Shift: 3:00 PM – 11:30 PM
- Overnight Shift: 11:00 PM – 7:30 AM
- Weekend Shifts Available
Qualifications:
- Must meet Behavioral Health Practitioner requirements under MN Statute 245I.04, subd. 4.
- Minimum of 4,000 hours of experience providing mental health services OR a related degree with required experience.
- Experience working in an IRTS, residential, or mental health setting preferred.
- Strong documentation and organizational skills.
- Ability to work independently and as part of a team.
- Must have a valid driver’s license and pass a background check.
