American Traveler is seeking an experienced RN for a Home Health position in Hawaiʻi, requiring at least 1 year of home health experience and OASIS documentation proficiency.

Home health setting serving adult patients in the field on the island of Hawaiʻi (Big Island)
-
1:1 patient ratio
-
Day shift, 5x8-hour shifts (8:00–16:30)
-
Productivity expectation of 25 units per week
-
No on-call requirements
-
Occasional floating to cover other Hawaiian islands (Oahu, Maui, and/or Kauai) may be required; all float-related expenses are covered

OASIS documentation system used; experience required
-
MatrixCare (HCHB) documentation experience preferred
-
Active RN license in the state of Hawaii, required and must be in hand

Current BLS certification (AHA)

Minimum 1 year of home health nursing experience required

More than 3 months of OASIS documentation experience required

Travel experience is not required

Required clinical skills include: adult home health, admission assessment, case management, discharge planning, recertification, resumption of care, routine visits, start of care, significant change in condition, transfer, wound care, wound V.A.C. management, pressure ulcer prevention/staging, drain management, IV management, phlebotomy, blood/blood product administration, peripheral IV/central line/port care, G-tube and NG tube management, colostomy/ileostomy care, suprapubic and urinary catheter insertion/management, post AV fistula/shunt care, blood glucose/glucometer management, nutritional assessment, medication assessment/management, pain assessment/management, chemotherapy side effect monitoring, port insertion/care, cast/brace/immobilizer device care, post-amputation care, post total joint replacement care, oxygen safety/administration, infection control, and clinical assessment documentation

Preferred skills include: psych home health, tracheostomy care, and HCHB documentation

Position involves providing in-home nursing care to adult patients, including starts of care, recertifications, and routine visits for Medicare and other patients

Scrubs in any color and closed-toed shoes are required

For consideration, be prepared to provide total months of OASIS start-of-care experience and your average frequency of Medicare patient starts of care per week
