Wound Nurse – RN/LPN
We are seeking a compassionate and clinically skilled RN/LPN Wound Nurse to join our healthcare team. In this role, you will be responsible for assessing, planning, implementing, and monitoring wound care treatments to promote healing and improve residents' quality of life. The ideal candidate is detail-oriented, knowledgeable in current wound care practices, and passionate about providing exceptional care to seniors and individuals in a skilled nursing environment. Working collaboratively with physicians, nursing staff, and interdisciplinary team members, the Wound Nurse plays a vital role in ensuring optimal clinical outcomes and maintaining the highest standards of resident care.
Job Functions
- Conduct skin assessments, wound evaluations, and weekly wound rounds.
- Assess, measure, stage, monitor, and document wounds accurately and timely.
- Perform wound treatments and dressing changes within nursing scope of practice.
- Communicate wound changes, infection concerns, and treatment needs to providers and nursing leadership.
- Ensure wound care orders are current and followed according to facility policy.
- Assist with pressure injury prevention, including repositioning, offloading, skin protection, and monitoring high-risk residents.
- Update care plans, wound records, treatment documentation, and progress notes.
- Provide education and support to nurses and CNAs regarding wound prevention, treatment procedures, documentation, and reporting changes.
- Participate in quality assurance, infection prevention, and survey readiness activities.
- Communicate professionally and compassionately with residents and families regarding wound status and care plans, as appropriate.
- Collaborate with the interdisciplinary team to develop and update individualized wound care plans.
Knowledge/Skills/Abilities
- Skilled nursing, long-term care, wound care, or post-acute care experience preferred.
- Strong clinical assessment and documentation skills.
- Ability to communicate effectively with providers, residents, families, nursing staff, and interdisciplinary team members.
- Strong attention to detail and ability to follow through on clinical concerns.
- Knowledge of pressure injury prevention, wound staging, infection control, and resident-centered care practices preferred.
- Dependable, organized, professional, and committed to quality resident care.
Employment Requirements
- Must possess an active, unencumbered, Licensed Practical Nurse or Registered Nurse license in the state of Arizona.
- Wound/Ostomy Certification preferred, or must be willing and able to obtain certification within a timeframe established by the facility.
- Valid Fingerprint Clearance Card (we will help you apply)
- Current TB test (available on-site)
- Current in-person CPR/AED Certificate for Healthcare Professionals.
