Position Summary:
As a contracted Physical Therapist, you will provide Outpatient therapy services, within the home to a diverse population of patients. Your focus will be on helping individuals restore mobility, improve strength and endurance, and achieve optimal physical functioning in their home environment.
Responsibilities:
- Perform initial assessments and create individualized care plans
- Deliver skilled PT interventions in accordance with physician orders and patient needs
- Reassess progress and adjust treatment plans accordingly
- Educate patients and caregivers on exercises and safe mobility techniques
- Document care and progress notes
- Collaborate with the multidisciplinary team to ensure coordinated care
