What You’ll Do:
- Leverage your existing network across hospital discharge planners, SNFs, assisted living facilities, physician offices, and care managers.
- Build and grow new relationships with hospital discharge planners, SNF case managers, physicians and other healthcare providers.
- Educate healthcare providers on the agency’s services and the value we bring to their patients.
- Own the growth of our referral base—with full internal support from our operations and clinical teams.
- Represent our agency with the confidence and credibility of someone who knows the home health game inside and out.
- Work independently while being backed by a responsive and agile leadership team that knows how to get things done.
- Develop the company’s marketing strategies, campaigns and programs that align with market analysis and company targets.
What We’re Looking For:
- Proven experience in Home Health or Hospice marketing (minimum 3 years—no exceptions).
- Established referral relationships and deep knowledge of the Sacramento healthcare landscape.
- A self-starter who thrives on results, relationship-building, and getting to "yes."
- A marketing professional who embodies professionalism, brings determination and creativity to the table, and who communicates with clarity and impact.

Pay: $100,000.00 - $150,000.00 per year
