Family Medicine Physician (MD/DO) – Primary Care & WellnessOverview
Synergy Health 360 is seeking a compassionate, patient-focused Family Medicine Physician (MD/DO) to join our growing team in Scottsdale and Phoenix, Arizona. We are looking for a physician who is passionate about delivering exceptional primary care while embracing preventive medicine, wellness, and evidence-based treatments that improve long-term patient outcomes.
The ideal candidate is committed to building lasting relationships with patients, managing acute and chronic conditions, and collaborating with a multidisciplinary team to provide comprehensive, high-quality care. Physicians interested in expanding into wellness, regenerative medicine, weight management, hormone optimization, and aesthetic medicine are encouraged to apply. Training will be provided where appropriate.
Responsibilities
- Provide comprehensive primary care services for patients of all ages.
- Perform detailed medical histories, physical examinations, and preventive health screenings.
- Diagnose and treat acute and chronic medical conditions using evidence-based practices.
- Develop individualized treatment plans, including medications, lifestyle modifications, and preventive care.
- Manage chronic diseases such as diabetes, hypertension, hyperlipidemia, obesity, thyroid disorders, and asthma.
- Order, interpret, and review laboratory tests, imaging studies, and diagnostic procedures.
- Perform office-based procedures including suturing, incision and drainage, joint injections, skin biopsies, cryotherapy, immunizations, and wound care.
- Educate patients on nutrition, exercise, preventive health, and disease management.
- Accurately document patient encounters using the practice's Electronic Medical Record (EMR) system.
- Collaborate with specialists and healthcare professionals to coordinate comprehensive patient care.
- Participate in quality improvement initiatives and help maintain clinical excellence.
- Maintain compliance with HIPAA and all applicable healthcare regulations.
- Support the practice's mission of integrating primary care with wellness and preventive healthcare services.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school.
- Completion of an accredited Family Medicine residency program.
- Active Arizona Medical License (or eligibility to obtain one before employment).
- DEA registration and unrestricted prescribing privileges.
- Board Certified or Board Eligible in Family Medicine.
- Minimum 2 years of outpatient primary care experience preferred.
- Experience with EMR/EHR systems.
- Excellent clinical judgment and diagnostic skills.
- Strong interpersonal, communication, and patient education abilities.
- Ability to work collaboratively in a fast-paced multidisciplinary environment.
- Interest or experience in preventive medicine, weight management, regenerative medicine, hormone replacement therapy, or aesthetic medicine is a plus.
Preferred Qualifications
- Experience with value-based care and preventive medicine.
- Experience performing in-office procedures.
- Bilingual (English/Spanish) is a plus.
- Interest in expanding clinical skills in wellness and regenerative medicine.
