What You'll Do
As a Home Health Physical Therapist, you'll help patients recover safely and comfortably in the place they want to be most: their home.
You will:
- Perform comprehensive physical therapy evaluations
- Develop individualized treatment plans
- Improve strength, mobility, balance, endurance, and functional independence
- Reduce fall risk and prevent hospital readmissions
- Assess home safety and recommend modifications
- Educate patients and caregivers
- Collaborate with nurses, occupational therapists, speech therapists, physicians, and case managers
- Participate in care planning and discharge coordination
- Complete timely electronic documentation
What We're Looking For:
- Graduate of an accredited Physical Therapy program
- Active Colorado Physical Therapist license
- Current CPR/BLS certification
- Valid driver's license and reliable transportation
