Duties
- Conduct comprehensive psychosocial assessments to evaluate patients' emotional, social, financial, and environmental needs within the home setting.
- Develop individualized care plans that address barriers to care, treatment adherence, caregiver support, and access to community resources.
- Collaborate closely with physicians, nurses, therapists, case managers, and community agencies to coordinate comprehensive patient care.
- Provide counseling, emotional support, crisis intervention, and coping strategies for patients and families dealing with illness, disability, chronic conditions, or life transitions.
- Assist patients in accessing community resources, including housing assistance, transportation services, financial aid programs, meal services, mental health resources, and caregiver support programs.
- Educate patients and families regarding available services, healthcare benefits, advance care planning, and long-term care options.
- Advocate for patients' rights, preferences, and access to appropriate healthcare and community services.
- Participate in discharge and transition planning to ensure continuity of care and successful community integration.
- Assess home environments for social and safety concerns and recommend interventions that support patient independence and well-being.
- Maintain accurate and timely documentation of assessments, care plans, interventions, and patient progress in compliance with agency policies and regulatory requirements.
Experience & Qualifications
- Bachelor's degree in Social Work (BSW) required; Master's degree in Social Work (MSW) preferred.
- Current state licensure or certification as a Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or equivalent credential as required by state regulations.
- Previous experience in home health, hospice, healthcare, rehabilitation, case management, or community-based services preferred.
- Strong knowledge of healthcare systems, Medicare, Medicaid, community resources, and patient advocacy practices.
- Excellent communication and interpersonal skills with the ability to establish trust and rapport with patients, families, and interdisciplinary team members.
- Demonstrated ability to manage sensitive situations with empathy, professionalism, and confidentiality.
- Experience with electronic medical records (EMR) systems and healthcare documentation standards.
- Ability to work independently, prioritize tasks effectively, and manage a flexible schedule within a community-based setting.
