Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
- Monitor patients’ vital signs, observe for changes in health status, and report findings
- Prepare nutritious meals tailored to individual dietary needs and assist with medication administration following prescribed care plans.
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces.
- Support clients with mobility challenges or disabilities through safe transfer techniques and assistive devices, ensuring their comfort and safety.
- Document care activities accurately in compliance with HIPAA regulations and agency policies; update care plans regularly based on client needs.
- Provide emotional support and companionship to promote mental well-being, especially for clients with dementia, Alzheimer’s, or other memory-related conditions.
Qualifications
- Proven experience in home health care, assisted living, nursing home environments, or group homes is highly preferred.
- Certification in CPR and First Aid is preferred but not required; additional training in Alzheimer's care, dementia care, hospice care, or working with individuals with developmental disabilities is a plus.
- Knowledge of HIPAA regulations to ensure patient confidentiality at all times.
- Ability to perform heavy lifting and physical tasks safely when assisting clients with mobility or transfers.
- Strong observational skills for patient monitoring and recognizing changes in health status.
- Experience working with individuals with autism or disabilities is advantageous.
- Excellent communication skills to collaborate effectively with clients, families, and healthcare teams.
