- Monitor and report patient condition to the interdisciplinary team and physician
- Participate in quality improvement initiatives to improve patient satisfaction and outcomes
- Collaborate with other healthcare professionals to ensure seamless transition of care

Qualifications:
Education: Associate’s Degree in Nursing (ADN) or Bachelor’s Degree in Nursing (BSN) from an accredited program.
Licensure: Current and valid Registered Nurse (RN) license in New York HERE. • Experience:1 year of experience in a skilled nursing or long-term care setting preferred, but not required
Certifications: Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification preferred.
