Overview
We are seeking an energetic and dedicated Wellness Director/ Delegating Nurse to lead and coordinate comprehensive wellness programs within our community. This vital role involves overseeing health services, managing interdisciplinary teams, and ensuring the delivery of exceptional care tailored to residents’ needs. The Wellness Director will champion a holistic approach to health, fostering an environment of compassion, safety, and continuous improvement. Responsibilities
- Develop, implement, and oversee resident-centered care plans that align with individual health goals and medical requirements.
- Supervise nursing staff, including nurse management and care team members, ensuring high-quality service delivery across all shifts.
- Monitor patient health status through regular assessments, documentation review, and utilization of EMR (Electronic Medical Records) systems to ensure accurate medical records management.
- Coordinate case management efforts for residents requiring specialized services such as hospice care, home health, or acute care.
- Ensure compliance with HIPAA regulations by safeguarding patient confidentiality and managing sensitive medical records appropriately.
- Oversee shift management to ensure adequate staffing levels, efficient workflow, and timely response to resident needs.
- Manage departmental budgeting, resource allocation, and operational efficiency while maintaining high standards of care quality.
- Lead staff training initiatives focused on physiology knowledge, medical terminology, anatomy fundamentals, and documentation review procedures.
- Collaborate with interdisciplinary teams to develop care plans that meet Medicare requirements and support residents’ well-being.
- Foster a positive environment that emphasizes leadership development, supervising experience, and continuous staff education.
Skills
- Extensive critical care experience combined with a strong background in assisted living, home health, hospice care, or acute care settings.
- Proven management skills with supervising experience in healthcare environments; adept at shift management and team leadership.
- Proficiency in EMR systems for accurate documentation review and medical records management.
- Deep understanding of nursing practices, medical terminology, physiology, anatomy knowledge, and patient monitoring techniques.
- Familiarity with HIPAA regulations and best practices for safeguarding protected health information (PHI).
- Experience in case management processes involving care plans development and coordination of services like Medicare or home care programs.
- Strong organizational skills with the ability to handle budgeting responsibilities effectively while maintaining high standards of resident care.
