About Peer Connect
Peer Connect LLC is a community-based behavioral health and peer support organization serving individuals and families through recovery-focused, client-centered services. We work with clients, referral sources, community partners, and internal care teams to help connect individuals with appropriate services, supports, and resources.
We are seeking a dependable, professional, and motivated Outreach Coordinator to support outreach efforts in the Colorado Springs area. This position is ideal for someone who is organized, comfortable working independently in the field, and able to build positive relationships with clients, families, providers, and community partners.
Position Summary
The Outreach Coordinator is responsible for conducting field-based outreach, following up on referrals, engaging clients, supporting eligibility communication, maintaining accurate documentation, and helping connect individuals to appropriate Peer Connect programs and services.
This is a full-time, 100% travel-based position. The Outreach Coordinator will spend the majority of their work time in the community, traveling to meet clients, referral sources, community partners, and other service-related contacts throughout Colorado Springs and surrounding service areas. This role requires strong time management, reliable transportation, consistent follow-up, and the ability to work professionally outside of a traditional office setting.
Key Responsibilities
The Outreach Coordinator will be expected to:
- Build and maintain positive relationships with clients, families, community partners, referral sources, and internal team members.
- Conduct outreach activities in the community and follow up on referrals in a timely and professional manner.
- Travel throughout the assigned service area to complete outreach, engagement, check-ins, referral follow-up, and community partner communication.
- Communicate clearly with clients regarding program eligibility, service availability, insurance-related requirements, and reimbursement limitations.
- Maintain accurate and timely documentation of outreach efforts, client contacts, referral follow-up, eligibility conversations, and program outcomes.
- Track outreach activity, engagement progress, barriers, and follow-up needs.
- Respond to calls, texts, emails, and internal requests within established timeframes.
- Proactively identify and communicate barriers that may affect client engagement or outreach goals.
- Attend required meetings, trainings, supervision sessions, and program check-ins.
- Follow all organizational policies, confidentiality requirements, HIPAA expectations, documentation standards, and compliance procedures.
- Meet assigned outreach, engagement, productivity, and follow-up goals.
- Represent Peer Connect professionally in the community.
Client Eligibility and Reimbursement Policy
The Outreach Coordinator must clearly and consistently communicate Peer Connect’s eligibility and reimbursement-related policies to clients and referral sources.
Clients who do not have active insurance coverage, or whose coverage cannot be verified for applicable services, may not be eligible for reimbursement through outreach-related programs or services. The Outreach Coordinator is responsible for communicating this policy professionally, documenting all eligibility-related conversations, and notifying the appropriate supervisor or team member when eligibility concerns, coverage issues, or service barriers arise.
Full-Time and Travel Expectations
This is a full-time position that requires consistent availability, reliable attendance, and regular communication with supervisors and team members.
This position requires 100% local travel. Candidates must be comfortable working in the field, managing their schedule, traveling between appointments and community locations, and completing documentation accurately and on time.
Candidates must have:
- Reliable transportation.
- A valid driver’s license.
- Current auto insurance.
- Ability to travel throughout Colorado Springs and surrounding service areas.
- Ability to maintain communication while working in the field.
- Ability to manage appointments, follow-ups, and documentation without constant direct supervision.
Mileage reimbursement, travel procedures, and any approved work-related expenses will be handled according to Peer Connect policy.
Required Qualifications
- High school diploma or GED required.
- Strong communication skills, both verbal and written.
- Ability to work independently and manage time effectively in a field-based role.
- Strong follow-up, organization, and documentation skills.
- Ability to maintain professional boundaries and confidentiality.
- Ability to communicate respectfully with clients from diverse backgrounds and life experiences.
- Comfortable discussing eligibility, insurance-related requirements, and service limitations with clients and referral sources.
- Basic computer, phone, email, and documentation system skills.
- Willingness to complete required onboarding, compliance, and program training.
- Must be able to pass required background check and onboarding requirements.
Preferred Qualifications
- Experience in outreach, peer support, behavioral health, case management, recovery services, reentry services, youth/family services, community navigation, or human services.
- Familiarity with Medicaid, behavioral health services, community-based referrals, or recovery support programs.
- Experience working with individuals impacted by substance use, mental health challenges, justice involvement, housing instability, or other barriers.
- Bilingual ability is a plus but not required.
- Peer support, recovery, CPR/BLS, Narcan, HIPAA, ethics, or related training is a plus.
Compensation and Performance Review
The starting pay rate for this Outreach Coordinator position is $22.00 per hour.
Because this role may be offered to a candidate who is newer to direct outreach coordination, compensation will be reviewed after the first 90 days of employment. During the review period, Peer Connect will evaluate performance, initiative, reliability, communication skills, documentation quality, follow-up consistency, professionalism, and achievement of assigned outreach goals.
Based on successful performance and demonstrated growth in the role, compensation may be reviewed and renegotiated at that time.
Performance Review Schedule
Peer Connect uses a structured early review process for this position:
30-Day Review: Assessment of onboarding progress, training completion, attendance, communication, and initial performance.
60-Day Review: Evaluation of outreach activity, follow-up consistency, documentation, communication effectiveness, and adherence to procedures.
90-Day Review: Comprehensive performance evaluation, including productivity, professionalism, client engagement outcomes, documentation quality, policy compliance, and consideration for compensation adjustment.
Follow-Up and Accountability
The Outreach Coordinator is expected to provide regular status updates, respond to inquiries within established timeframes, and proactively communicate any barriers that may affect outreach goals.
Consistent follow-up with clients, referral sources, community partners, and internal team members is a key requirement of this position. Failure to meet performance expectations, maintain accurate documentation, communicate appropriately, or comply with reimbursement and eligibility policies may result in corrective action and may affect eligibility for compensation increases.
Work Environment
This is a community-based field position. Work may take place in community settings, client meeting locations, partner agencies, office locations, and other approved service-related environments. The Outreach Coordinator must be able to travel regularly, communicate professionally in the field, and complete required documentation in a timely manner.
