We are looking for someone who can transport our consumers safely to and from their destinations, and work together to enhance their daily lives.
*Please note that this a part-time position only.*
Roles and responsibilities include:
- Provide and/or supervise the provision of one-on-one instruction on job site of assigned consumers.
- Develops resources and contacts to facilitate provision of Supportive Employment services.
- Completes required documentation and monitors consumers files.
- Attends team meetings in accordance with staffing schedules.
- Provide transportation for consumer to activities and appointments.
- Encourage and facilitate cooperation, pride, trust, and group identity; fosters commitment and team spirit; work cooperatively with others to achieve goals.
- Works and communicates with the general public, internal and/or external customers to provide information and quality services.
HS Diploma or GED and one (1) year experience providing social or direct care services to special needs persons required. Moderate lifting may also be required. Must have valid driver's license and clean driving record along with valid vehicle insurance. Must be able to pass drug screen and criminal background check.
