Job Overview
Nurture Home is seeking a dedicated and compassionate In-Home Health Risk Assessment Nurse to join our dynamic healthcare team. In this vital role, the nurse is responsible for conducting comprehensive in-home assessments for prospective and current clients. The nurse evaluates clients' physical, mental, and psychosocial needs, develops individualized care plans, and ensures services are delivered in accordance with agency policies, state regulations, and professional nursing standards.
Key Responsibilities
- Conduct initial and ongoing nursing assessments in clients' homes.
- Evaluate clients' medical conditions, functional abilities, medication regimens, and home environments.
- Develop and update individualized plans of care based on assessment findings.
- Identify health risks, safety concerns, and needed interventions.
- Coordinate with physicians, caregivers, care managers, and agency staff to ensure continuity of care.
- Provide client and family education regarding health conditions, medications, and care needs.
- Complete all required assessment documentation accurately and within established timeframes.
- Monitor clients' progress and recommend modifications to care plans as needed.
- Ensure compliance with state, federal, and agency regulations.
- Participate in case conferences and interdisciplinary team meetings.
- Report significant changes in client condition to appropriate healthcare providers and agency leadership.
Qualifications
- Current, unrestricted nurse license in Pennsylvania.
- At least one year of nursing experience preferred; training will be provided for qualified candidates without prior professional experience.
- Strong assessment and clinical documentation skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation, a valid driver's license, and automobile insurance.
- Current CPR certification.
Physical Requirements
- Ability to travel to client homes within the service area.
- Ability to lift to 25 pounds occasionally.
- Ability to perform nursing assessments in various home environments.
Compensation
- Per-assessment rate or hourly rate (to be determined by the agency).
- Mileage reimbursement for client visits.
