Position Summary
A-Z Nursing Services, Inc. is a growing, client-centered home health agency seeking a dedicated and flexible Licensed Practical Nurse (LPN) to support both clinical services and office operations. This hybrid role is ideal for an LPN who is organized, able to multitask effectively, and willing to assist with care coordination and administrative functions as the agency continues to grow. The LPN functions under the supervision of a Registered Nurse (RN).
Key ResponsibilitiesClinical Responsibilities
- Perform Home Health Aide (HHA) supervisory visits in accordance with agency policy and regulatory requirements
- Assist with coordination of care and communication with the interdisciplinary team
- Support clinical staff with client-related needs, follow-up, and documentation
Administrative & Office Support
- Perform miscellaneous administrative tasks as needed to support daily operations
- Assist with scheduling of staff and coordination of client visits
- Support general office duties including data entry, filing, and communication
On-Call Coordination
- Participate in every other weekend and weekday on-call rotation
- Manage incoming calls, referrals, and urgent scheduling needs
Passport / Waiver Program Support
- Assist with Passport client management and coordination of services
- Maintain communication with case managers and ensure timely follow-up
HR, Compliance & Attendance Support
- Assist with onboarding and maintain accurate employee personnel files, ensuring all required documentation is complete
- Track licenses, certifications, and compliance requirements while supporting adherence to agency policies and regulatory standards
- Assist with audit preparation and documentation review, including monitoring attendance and reporting concerns to leadership
Qualifications
- Current Licensed Practical Nurse (LPN) license in the state of Ohio
- Home health experience preferred
- Strong organizational skills with the ability to multitask, prioritize workload, and efficiently switch between tasks in a fast-paced environment
- Must be highly adaptable and able to shift focus quickly based on agency needs
- Excellent communication and teamwork abilities
- Ability to work independently and adapt to changing needs
- Must be willing and able to travel within Tuscarawas County
- Reliable transportation required
Schedule
- Full-time position
- Includes on-call rotation, variable based off current staff/census
