Job Title: Registered Nurse – Home Health
Location: Boise, Idaho
Job Duration: 13 Weeks
Start Date: ASAP
Shift: Day Shift (8:00 AM – 5:00 PM), Monday–Friday
Hours: 40 per week
"Navitas Healthcare, LLC" is seeking an experienced Home Health RN to provide skilled nursing care in patients’ homes. The role requires strong independence, clinical judgment, and experience in home health documentation and OASIS assessments.
- Provide direct skilled nursing care in home settings
- Complete comprehensive patient assessments
- Perform OASIS documentation and accurate charting
- Manage individualized care plans for patients
- Administer medications and monitor patient responses
- Provide wound care, IV therapy, and post-acute support
- Educate patients and families on care plans and disease management
- Coordinate with physicians and interdisciplinary teams
- Maintain compliance with home health regulations and protocols
- Active RN License
- Minimum 1–2 years Home Health experience required
- Strong experience with OASIS documentation (mandatory)
- Experience with Homecare Homebase system (required)
- Ability to manage productivity expectations (approx. 30 points/week)
- Strong communication and independent working skills
