Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans.
- Administer medications via injections, IV infusions, and other routes, ensuring adherence to medication protocols and safety standards.
- Manage complex wound care procedures such as dressing changes and catheterization with precision and Aseptic technique.
- Provide specialized care for patients with chronic conditions like Alzheimer’s disease, dementia, or memory impairments, supporting their unique needs.
- Assist with diagnostic evaluations including specimen collection, processing, and documentation within EMR/EHR systems like Epic or Cerner.
- Educate patients and their families on health management strategies, medication administration, nutrition, and safety protocols.
- Coordinate discharge planning and case management to ensure seamless transitions between care settings while complying with long-term care regulations.
Experience
- Valid Licensed Practical Nurse (LPN) license with current state certification.
- Proven experience working in home health or community-based settings; experience in skilled nursing facilities or hospitals is highly valued.
- Critical care experience such as ICU or emergency medicine enhances your ability to manage acute situations confidently.
- Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth for accurate medical documentation.
- Knowledge of ICD-9/10 coding standards and medical terminology for precise recordkeeping and billing processes.
- Experience working with diverse patient populations including geriatric patients, individuals with developmental disabilities, or those requiring hospice or palliative care.
- Strong skills in patient assessment, triage protocols, pain management—including acute pain—and infection control practices.
