Cardiac Registered Nurse (RN) – Home Health
Florida State Nurse Registry, LLC is seeking an experienced and compassionate Cardiac Registered Nurse (RN) to provide skilled nursing services to patients with cardiovascular conditions in a home health setting. This position focuses on patient assessment, disease management, education, and coordination of care to support positive patient outcomes and reduce hospital readmissions.

ResponsibilitiesClinical Care
- Perform comprehensive nursing assessments with a focus on cardiovascular conditions, including CHF, CAD, post-MI recovery, arrhythmias, and hypertension.
- Monitor vital signs, cardiac symptoms, and changes in patient condition.
- Administer medications and treatments as prescribed.
- Assess for complications such as fluid overload, chest pain, edema, and shortness of breath, and intervene appropriately.
- Provide skilled nursing services and wound care when applicable.
- Develop and implement individualized care plans.
Care Coordination
- Collaborate with physicians, cardiologists, case managers, and interdisciplinary care teams.
- Communicate significant changes in patient condition promptly.
- Maintain accurate and timely documentation in accordance with agency, state, and federal guidelines.
- Support continuity of care and assist in preventing avoidable hospitalizations.
Patient & Caregiver Education
- Educate patients and families on:
- Cardiac disease processes
- Medication management and adherence
- Low-sodium diets and fluid restrictions
- Lifestyle modifications and cardiac rehabilitation principles
- Early recognition of worsening symptoms
- Reinforce physician instructions and discharge plans.
QualificationsRequired
- Active and unrestricted Florida Registered Nurse (RN) License.
- Minimum 1–2 years of nursing experience.
- Experience in cardiac care, acute care, home health, or related clinical settings.
- Current BLS Certification.
- Reliable transportation and ability to travel to patient homes.
- Strong clinical assessment, critical thinking, and documentation skills.
Preferred
- Home Health experience.
- OASIS documentation experience.
- ACLS Certification.
- Experience with remote patient monitoring or telehealth.
- Knowledge of chronic disease management and readmission prevention strategies.
