Job Summary
New Era Mental Health is seeking a compassionate, detail-oriented Case Manager to join our team. The Case Manager will assess client needs, develop individualized service plans, coordinate community resources, and provide ongoing support to help clients achieve stability and improved quality of life. This role is full-time and in-person, and requires strong communication, advocacy, organizational skills, and familiarity with social service and healthcare systems in King County (or demonstrated ability to work effectively in similar systems).
Key Responsibilities
- Conduct comprehensive intake and ongoing assessments to determine client needs and eligibility for services
- Develop individualized service plans in collaboration with clients and care teams
- Coordinate referrals to appropriate community resources, treatment providers, and support services
- Monitor client progress and update service plans as needed to support goal attainment
- Maintain timely, accurate documentation in accordance with agency and regulatory standards
- Advocate for clients within healthcare, legal, and social service systems
- Collaborate with multidisciplinary teams, including clinicians, therapists, and social workers
- Respond to crises and provide support or appropriate intervention when needed
- Ensure compliance with privacy, confidentiality, and documentation requirements
- Perform other related duties as assigned
Required Qualifications
- Bachelor’s degree in Social Work, Psychology, Nursing, Human Services, or a related field
- AND
- Minimum of 2 years of experience in case management, social services, or a related role
OR
- Certified Peer Support Specialist credential (highly considered in lieu of degree)
AND
- Significant experience working within King County service systems or demonstrated equivalent experienceRequired Skills & Knowledge
- Strong knowledge of community resources and case management best practices
- Excellent interpersonal, communication, and advocacy skills
- Ability to manage multiple cases, prioritize tasks, and meet deadlines
- Proficiency with documentation and case management systems
- Valid driver’s license and reliable transportation (some field work may be required on a case-by-case basis)
Preferred Qualifications
- Case management experience within King County
- Experience working with diverse populations, including individuals experiencing homelessness, mental health challenges, and behavioral health needs
- Familiarity with Medicaid, Medicare, and other public benefit programs

Pay: $23.00 - $27.00 per hour
