Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy to help them navigate various challenges. This position requires a strong commitment to social work principles and a passion for making a positive impact in the community. This role will work individuals and their families affected by addiction and currently participating in problem solving courts.
Duties
- Conduct assessments to determine clients' needs and develop personalized service plans.
- Provide crisis intervention and support to clients in distress, ensuring their immediate safety and well-being.
- Facilitate access to resources such as behavioral health services, addiction counseling, childcare, and workforce development programs.
- Ensuring compliance with relevant regulations and best practices.
- Mentor clients through their journey, offering encouragement and guidance as they work towards their goals.
- Collaborate with community organizations, government agencies, and other stakeholders to coordinate services for clients.
- Maintain accurate records of client interactions, progress, and outcomes in accordance with agency policies and Medicaid.
- Advocate for clients’ rights and needs, including knowledge of housing assistance programs in Lancaster County.
Experience
Candidates should possess the following skills and experience:
- Background in social work or related fields is preferred.
- Familiarity with crisis intervention techniques and behavioral health resources.
- Experience working with behavioral health in various capacities is beneficial.
- Strong mentoring skills to support client growth and development.
- High level of integrity as you will be expected to role model health recovery
- Professional and/or lived experience with addiction is a must.
- If you are currently active in Recovery a minimum of 2 years is a must

Job Type: Full-time
Pay: $19.50 - $21.50 per hour
