Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health through observation of vital signs, medication administration, and recognizing changes in condition
- Support clients with medical procedures such as catheter care, wound care, and managing medical devices using EMR (Electronic Medical Records) systems
- Facilitate safe patient transfers utilizing proper patient transfer methods and equipment like Hoyer lifts or transfer belts
- Maintain accurate medical documentation, logs, and records while adhering to HIPAA regulations and home healthcare standards
- Provide companionship and emotional support, especially for clients with dementia or experiencing end-of-life care needs
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment
Skills
- Proven experience working with seniors or individuals with disabilities in home healthcare or assisted living settings
- Strong knowledge of medical terminology, medication management, and vital signs monitoring
- Skilled in patient observation, patient personal care assistance, and client communication
- Familiarity with EMR/EHR systems for documentation and medical records management
- Ability to perform CPR and First Aid confidently; certification preferred
- Experience working with geriatric patients, dementia care, Alzheimer's care, hospice care or memory care is highly desirable
- Knowledge of home health care regulations, HIPAA compliance, and confidentiality standards
- Physical ability to lift heavy objects safely and assist with patient transfers using appropriate techniques

Pay: $19.00 - $20.00 per hour
