Reliant Health VA, LLC is a licensed In Home Support Services (IHSS) provider serving individuals with Intellectual and Developmental Disabilities (IDD).

What You’ll Do
- · Provide personal care assistance including bathing, grooming, dressing, oral hygiene, and toileting
· Assist with safe interactions in the community
Support teenage boy with behavioral support needs
· Implement goals and activities outlined in the Individual Support Plan (ISP)
· Assist with meal planning and preparation in accordance with dietary needs and preferences
· Transport individuals to medical appointments, community outings, and activities
· Document all services accurately and on time using the Electronic Visit Verification (EVV) system
· Report changes in health, behavior, or safety concerns to the Program Director promptly
· Respond to incidents and emergencies per DBHDS regulations and agency procedures
· Maintain confidentiality and uphold individual rights per OHR 12VAC35-115 at all times
· Participate in required trainings, team meetings, and supervision sessions
· Model community integration, independence, and self-advocacy
Must‑Haves
- 3+ year experience with IDD or related care.
- Valid driver’s license, reliable vehicle, and active auto insurance.
- Able to pass TB screening, background checks (Fieldprint), OIG/LEIE.
- CPR/First Aid (or willing to obtain)
- Reliable personal transportation
- Clean driving record (MVR check required)
- Must have experience working with individuals with behavioral support needs
Schedule (Part Time)
Monday- Friday 5pm-9pm
