Registered Nurse (RN) – Home Care Services: Help Clients Thrive at Home
Griswold Home Care is looking for a compassionate and experienced Registered Nurse (RN) to join our team. If you enjoy providing personalized care, working independently, and making a meaningful impact in your community, we'd love to meet you.
As an RN, you'll play a key role in supporting our clients and caregivers by completing clinical assessments, coordinating care, and helping ensure every client receives high-quality, individualized services that allow them to remain safely in their own homes.
What You'll Do
- Conduct initial nursing assessments and periodic client reassessments.
- Develop and update individualized plans of care based on each client's needs.
- Monitor client progress and identify changes that require follow-up or intervention.
- Provide clinical guidance and support to caregivers and office staff.
- Educate clients and family members on health concerns, safety, and care recommendations.
- Communicate with physicians and other healthcare professionals to coordinate services.
- Maintain thorough, accurate, and timely nursing documentation.
- Ensure all services are provided in accordance with nursing standards and company policies.
Qualifications
- Active North Carolina Registered Nurse (RN) license in good standing.
- Two or more years of nursing experience preferred.
- Home care, home health, hospice, or case management experience is a plus but not required.
- Excellent assessment, communication, and problem-solving skills.
- Ability to prioritize responsibilities and work independently.
- Reliable transportation and a valid driver's license are required.
