Your Role as an RN Case Manager:
You’ll play a key role in delivering high-quality care to patients in their homes and helping them live healthier, more independent lives. From assessments to care planning to coordinating with families and physicians—you’ll be at the heart of it all.
Key Responsibilities:
- Complete initial and ongoing patient assessments
- Develop and adjust care plans tailored to each patient’s needs
- Administer medications and treatments
- Provide education and emotional support to patients and families
- Serve as a Case Manager to coordinate all aspects of care
- Communicate regularly with physicians and other care team members
What You’ll Need:
- Active RN license in Kansas (or multi-state license)
- Current CPR certification
- A big heart and a passion for home care
- New grads welcome—no experience required!
