Position Overview:
SafeinHome is seeking a compassionate, motivated, and detail-oriented Entry-Level Case Manager to join our team. In this role, you will work with a cross-functional internal team consisting of Remote Supports Consultants and Field Service Specialists, and serve as the internal operations point-person for supporting those whom we serve. In addition, you will interact regularly with individuals, their families, and interdisciplinary support teams to develop and manage personalized support plans that reflect each individual's goals, preferences, and abilities.
Key Responsibilities:
- Collaborate with SafeinHome personnel, clients, families, and support teams to assess needs and develop individualized support plans
- Monitor, evaluate, and adjust support plans to ensure ongoing alignment with client goals and progress
- Advocate for clients' rights to autonomy, independence, and self-determination
- Maintain accurate and up-to-date case notes, documentation, and reports
- Coordinate with service providers, agencies, and other stakeholders to ensure continuity and quality of support
- Participate in team meetings, training, and professional development activities
- Uphold ethical standards and regulatory compliance at all times
Qualifications:
- Passion for supporting individuals with disabilities and older adults
- Strong interpersonal, communication, and organizational skills
- Ability to work collaboratively in a team-oriented environment
- Detail-oriented with a commitment to maintaining accurate documentation
- Reliable transportation and punctuality is essential
- Proficiency in using Microsoft Word and Excel, Zoom, Teams, Outlook email, and other necessary virtual meeting platforms
- Experience with various web browsers, such as Internet Explorer, Microsoft Edge, Chrome, and Firefox.
- Must pass an FBI background check.
Preferred Qualifications:
- Associates degree or 5+ years’ experience in Human Services, Social Work, Psychology, or a related field
- Experience in a human services, case management, or support coordination role
- Knowledge of person-centered planning and support strategies
- Familiarity with Medicaid Waivers, state services, or related programs (a plus, not required)

Job Type: Full-time
Starting Pay: $24.00 - $26.40 per hour
Please note, the pay rate may vary depending on the shift worked. This position has a differential in place for shifts that may occur during the Morning, Evening, or Night. Please inquire about company needs in regard to shift placement during the interview.
