Duties
- Conduct comprehensive patient assessments to determine care needs, including vital signs, physiological status, and symptom management
- Develop and implement individualized hospice care plans in collaboration with interdisciplinary healthcare teams
- Manage medication administration and monitor patient responses to optimize comfort and symptom control
- Coordinate discharge planning, home health services, and post-acute care to ensure smooth transitions across settings
- Document all patient interactions accurately using Electronic Health Records (EHR) systems while maintaining HIPAA compliance
- Provide emotional support and education to patients and their families about disease progression, symptom management, and end-of-life options
- Collaborate with physicians, social workers, behavioral health specialists, and other healthcare professionals to deliver comprehensive hospice care
- Participate in case management activities including diagnostic evaluation, medical records review, and utilization management to ensure quality standards are met
Experience
- Hands-on experience with hospice, home health care, or outpatient clinics involving patient assessment.
- Familiarity with managed care policies, health regulation standards, and medical terminology relevant to hospice and palliative services
- Experience managing patients across diverse populations including geriatrics, Alzheimer's, and chronic illness.
- Proficiency in Electronic Health Records (EHR) management systems such as EMR/EHR platforms for documentation and communication purposes
- Strong understanding of physiology principles related to patient assessment and symptom management in complex cases
- Ability to collaborate effectively with multidisciplinary teams while demonstrating excellent client communication skills
