Gilead Health Care, Inc. is seeking dedicated Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) to provide high-quality home health services in South Austin and the surrounding areas. While our office is based in Round Rock, we are expanding our elite team to support the growing needs of patients in the South Austin region.
Core Responsibilities
- Skilled Nursing: Provide assessment, medication management, wound care (including Wound VAC), and IV nutrition/infusion therapy.
- Care Coordination: Develop and implement individualized Plans of Care (POC) in collaboration with physicians and interdisciplinary teams.
- Patient Education: Instruct patients and families on disease management and self-care techniques.
- Documentation: Maintain accurate records using electronic systems like Wellsky/Kinnser.
Schedule: Flexible Day Shift
Pay: Competitive and based on experience
Qualifications
- License: Valid Texas RN or LVN license.
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Experience: Minimum of 1 year of nursing experience (Home Health or related field preferred).
- Requirements: Reliable transportation, valid driver’s license, auto insurance, and strong interpersonal skills.
