JOB Under general supervision, performs work of moderate difficulty in developing and providing public health education, conducting outreach, and connecting community members to services; performs related work as assigned. This is a grant funded position, and dependent upon the availability of grant funds. EXAMPLE OF DUTIES

Conducts outreach to individuals, families, schools, and community organizations to promote healthy behaviors and program participation.

Delivers evidenced-based and culturally appropriate education sessions on topics such as adolescent health, substance use, naloxone, tobacco use prevention and cessation, chronic disease prevention, and emergency preparedness.

Assists in planning, coordinating, and evaluating program activities and educational events.

Provides support, and referrals to healthcare and social service providers as appropriate.

Establishes and maintains cooperative working relationships with schools, healthcare providers, community partners, and other stakeholders.

Maintains accurate records of outreach, education sessions, and client interactions; prepares required reports.

Identifies barriers to care and advocates for improved access to health and wellness services.

Participates in required trainings to maintain current knowledge of public health best practices and community resources.

Performs other related duties as assigned.
SUPPLEMENTAL INFORMATION High School diploma or G.E.D.; AND three years’ experience working in a health field with one year of general office experience; AND a valid driver's license.
