Join our compassionate healthcare team as a Hospice Evening/Night On-Call Nurse, where your expertise will provide vital support to patients and families during critical hours. Duties
- Respond promptly to after-hours patient calls, triaging symptoms and providing guidance based on clinical protocols.
- Conduct comprehensive patient assessments remotely or onsite, including vital signs, symptom evaluation, and medication review.
- Administer medications via IV infusion, injections, or other routes as prescribed, ensuring aseptic technique and safety standards.
- Collaborate with interdisciplinary teams to develop or adjust individualized care plans aligned with patients’ goals and comfort needs.
- Document all patient interactions accurately in electronic health records (EHR systems), maintaining compliance with HIPAA regulations.
- Provide emotional support and education to patients and families about disease progression, symptom management, and end-of-life care options.
- Coordinate emergency responses when necessary, including ventilator management or acute pain control, in collaboration with emergency services if required.
Qualifications
- Valid nursing license with current registration; experience in hospice & palliative medicine strongly preferred.
- Demonstrated proficiency with EMR/EHR systems and familiarity with medical documentation standards.
- Critical care experience including ICU or emergency medicine enhances your ability to handle complex cases under pressure.
- Experience working with individuals with disabilities, memory care needs, or in long-term care settings such as nursing homes or assisted living facilities is highly valued.
- Strong assessment skills including vital signs measurement, physical examinations, and symptom evaluation are required.
- Ability to work independently during evening/night shifts while maintaining excellent communication with daytime teams.

Pay: $27.00 - $40.00 per hour
