American Traveler is hiring an experienced RN for a Hospice Case Management position in Plymouth, MA, requiring an active MA RN license and prior hospice experience in an outpatient, community-based setting.

Outpatient hospice case management role with patient visits conducted in both private homes and care facilities
-
Expected to complete 4–5 patient visits per day
-
Daily travel of approximately 30–50 miles required to reach patients in the community

8-hour day shift, Monday through Friday, 8:00 AM – 4:30 PM

32 hours per week

Mileage reimbursed per IRS guidelines

Active MA RN license currently in hand

Current BLS certification required

Prior hospice nursing experience required

Ability and willingness to travel daily by personal vehicle to patient homes and facilities

Provide routine hospice case management visits to patients in home and facility settings

Pass/Fail testing required: Telemetry/ECG and Medication exams

COVID vaccine with booster and flu vaccine required (medical exemptions accepted)

Candidates must reside more than 50 miles from the facility — local candidates are not accepted

Candidates who have worked for Beth Israel Lahey Health (travel, permanent, or per diem) within the last 6 months are not eligible — a 6-month separation from any BILH facility is required
