Description:
Client Care Manager is responsible for directing, supervising, planning and evaluating the programs administered and assumes responsibility and accountability for operational activities assuring the highest quality, exceptional care and services for Assisting Hands Home Care West Orange.
In one of our most important roles as a Client Care Manager, you will work with an existing base of home care clients and their families, work with prospective clients to implement solutions and interface with case managers and discharge personnel at skilled nursing facilities, hospitals, physicians, etc. Best of all you will be helping families put long-term care solutions in place for seniors to promote independence, improve their quality of life and provide peace of mind for those that love them.
Education:
Associate degree (two-year college or technical school) Preferred
Qualifications:
- Work with a sense of urgency while maintaining a high level of detail and organization.
- Experience in senior service setting preferred.
- Must be a good problem solver and able to work independently.
- Exceptional communication skills (in person, on phone, and through email)
- Optimistic demeanor that conveys warmth, compassion and competence.
- Ability to gently persuade others that may be resistant to care but will ultimately benefit greatly from it.

- Bilingual (English/Spanish) is a plus!

Primary Duties:
· Responsible for all day-to-day operations of Assisting Hands Home Care West Orange and other areas assigned.
· Collaborates with office employees to coordinate and oversee all direct and indirect patient care and services provided.
· Ensures organizational compliance with state, federal and local laws and regulations.
· Ensures staff compliance with AHWO Employee handbook and EOS processes.
· Develops, maintains, updates and implements written policies and procedures for AHWO.
· Collaborates with the CEO to prepare and administer budgets for assigned operations.
· Maintains communication with AHWO employees through interdepartmental planning and collaboration.
· Participates in the recruitment and selection of personnel.
· Collaborates with the CEO to evaluate and revise systems, policies and procedures for community service operations to ensure the provision of adequate and appropriate care and services.
· Work with our RN and/or take inquiries from prospective clients to set up and perform in home assessments.
· A demonstrated interest in helping people in a health-related field.
· Ability to work as an advocate and leader in a team environment on behalf of clients.
· Understanding how successfully work with key referral sources.
· Team player that adds to the company culture and performance of the overall team through trust, competence and enthusiasm.
· Willingness and an eagerness to learn and grow.
· Overseeing matching of appropriate caregivers to cases based on personality, availability, and skill set.
· Overseeing that all cases are covered and arranges for substitute/back-up coverage as necessary.
· Provides information about services to prospective clients.
· Responsible for maintaining client files and keeping them up to date.
· Responsible for verifying authorization of services with payors.
· Serves as the point of contact for clients, caregivers and case managers.
· Counsels caregivers regarding attendance or performance issues.
Hours: 8:30 AM – 5:00 PM, Monday through Friday
