About Calhoun Congregate Living Facility/St Elizabeth Congregate:
- 24-bed Skilled Nursing Facility with a Congregate Living Facility
- Supportive interdisciplinary team with a strong clinical reputation
- Well-connected within the South Bay healthcare community
Your Role at a Glance:
- Build and nurture referral relationships with hospital discharge planners, case managers, physicians, and other providers
- Collaborate with admissions and clinical teams to ensure smooth and appropriate placements
- Actively manage the referral pipeline and follow up with professionalism and care
- Track and report performance metrics regularly—support is provided, but ownership is expected
- Represent Calhoun Congregate Living Facility/St Elizabeth Congregate: with authenticity, warmth, and professionalism
What We’re Looking For:
- 5+ years of skilled nursing or post-acute care marketing experience
- A strong referral network and proven ability to grow census
- Working knowledge of managed care, and HMO processes
- Excellent communication, organization, and follow-up skills
- A collaborative, goal-driven mindset—you’re self-motivated but team-oriented
