We need professionals to assist in helping us meet the needs of our clients.

● Provides services in a one-to-one setting for individuals experiencing a crisis situation with emphasis on linking the individual to services and coordinating care.
● Implement interventions and service plans.
● Models and teaches problem-solving techniques
● Coordinates and provides transportation to services and medical appointments as needed.
● Coordinates scheduling of appointments with the individual and family.
● Documents all service delivery in a timely manner as required by the Department of Behavioral Health and Developmental Services.
● Maintains regular and ongoing contact with referring agencies, as well as other agencies involved in delivering services to the individual and his/her family.
● Actively coordinates and plans discharge and aftercare plans.
● Attends interagency meetings and supervision sessions as necessary.
● Attends and participates in orientation, in-service training, and conferences as scheduled.
● Performs other duties as assigned.
