RN – Per Diem Pay Rates (DuPage County)
- Start of Care (SOC): $120
- Complex Visits (Recertification, ROC, Discharge, Wound Care): $75
- Routine Skilled Visit: $70
Rates reflect visit complexity and language-specific assignments.

- Complete SOC, Recertification, and Discharge visits (OASIS)
- Provide skilled nursing care (wound care, medication management, disease education)
- Coordinate care with physicians, PT/OT, and interdisciplinary team members
- Document accurately and timely in our EMR
- Build meaningful, one-on-one relationships with patients and families

Qualifications
- Active Illinois RN license
- OASIS experience required
- Minimum 1 year recent home health experience preferred
- English proficiency required; bilingual (Spanish preferred)
- Current CPR certification
- Reliable, insured vehicle + valid IL driver’s license
- Comfortable with smartphone, laptop, and EMR
- Strong clinical judgment and communication skills
Service Areas
We are actively seeking RNs in:
DuPage County, IL — including Naperville, Wheaton, Downers Grove, Lombard, Oak Brook, Lisle, Addison, Villa Park, Carol Stream, Westmont, Woodridge, Glen Ellyn, and surrounding communities.
Compensation Summary
- Pay per visit: $70–$120
- Rates vary by visit type, complexity, and assignment
