Job Description: The NJ Department of Human Services, Division of Disability Services is seeking social workers or health/human services-related professionals for case management for the Traumatic Brain Injury Fund (TBI), a program providing services and supports to individuals with a medically documented
TBI.
Job Responsibilities:
- Assess beneficiary's needs annually in person; quarterly by phone or virtually
- Design, implement and monitor individual service plans
- Counsel and aid individuals and families requiring assistance, as needed
- Complete documentation, including but not limited to data entry, progress notes, activity reports, time sheets and logs
- Maintain records according to professionally accepted standards
- Coordinate beneficiary's services with service provider(s), as needed
- Network with service providers to generate community resources for beneficiary's, as needed
Pay: $31.92 per hour
Work Location: In person
