VidaWell Care is a growing home health agency serving patients throughout Miami-Dade, delivering skilled, compassionate care that helps seniors and individuals with chronic or complex conditions stay safely and comfortably in their own homes. We're looking for a Medical Social Worker to join our interdisciplinary care team.
About the Role
As our Medical Social Worker, you'll be a key part of the care team, addressing the psychosocial, emotional, and practical needs that directly impact a patient's health outcomes and ability to remain safely at home. You'll work closely with our RNs, therapists, and care coordinators to make sure patients and families have what they need — beyond just the clinical care.
Responsibilities
- Conduct psychosocial assessments for patients newly admitted to home health services
- Identify community resources for patients and families (financial assistance, transportation, home modifications, caregiver support, etc.)
- Assist with discharge planning and coordination of long-term care needs
- Provide short-term counseling support related to illness adjustment, end-of-life concerns, or family dynamics affecting care
- Coordinate with physicians, RNs, and case managers to update the plan of care based on identified needs
- Complete timely, accurate documentation in compliance with Medicare/Medicaid home health regulations
- Educate patients and caregivers on available entitlement programs and community services
- Participate in interdisciplinary team meetings/case conferences as needed
Qualifications
- Master's Degree in Social Work (MSW) from a CSWE-accredited program (required)
- Minimum of 1 year of social work experience in a health care setting (required per Medicare Conditions of Participation)
- Active Florida license (LCSW) preferred, or license-eligible
- Bilingual English/Spanish strongly preferred
- Valid Florida driver's license, reliable transportation, and current auto insurance (this role requires travel to patients' homes across Miami-Dade)
- Ability to pass a Level 2 background screening
- Current CPR certification (or willingness to obtain)
- Strong communication, documentation, and time-management skills
- Comfortable working independently in the field while staying connected with the broader care team
