We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Assisted Living team. The LPN plays a key role in providing high‑quality nursing care, medication management, and clinical oversight while supporting residents’ independence, dignity, and quality of life.
Administer medications and treatments per physician orders and state regulations
Monitor residents’ health status and report changes to RN/Director of Nursing
Perform nursing assessments and document care accurately and timely
Provide direct nursing care and respond to medical needs and emergencies
Collaborate with caregivers, management, and healthcare providers
Supervise and support caregivers/medication aides as assigned
Ensure compliance with assisted living policies, procedures, and regulatory standards
Promote a safe, positive, and resident-centered environment
Active LPN license in the state of [State], in good standing
Prior experience in assisted living, long-term care, or geriatrics preferred
Knowledge of medication administration and documentation standards
Strong communication, organizational, and leadership skills
