Duties
- Assist individuals with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to promote independence and dignity
- Observe for changes in condition, and reporting findings accurately
- Support medication administration following HIPAA guidelines and care plans to ensure safety and compliance
- Prepare nutritious meals tailored to dietary needs, assist with feeding when necessary, and maintain cleanliness in the living environment
- Document care activities meticulously, including patient observations, progress notes, and incident reports
- Engage individuals in social activities and community outings to foster socialization and community integration
Requirements
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred
- Knowledge of HIPAA regulations to ensure confidentiality of patient information
- Certification in CPR and First Aid is required; additional certifications like Alzheimer's or dementia care are a plus
- Ability to lift heavy objects safely and perform physically demanding tasks as needed for patient care
- Strong observational skills for patient monitoring and recognizing changes in health or behavior
- Excellent communication skills to collaborate effectively with team members, clients, families, and healthcare professionals
- Flexibility to work various shifts including evenings, weekends, or overnight hours as needed
