Location: Various Client Homes – New York and New Jersey. Licenses for both New York and New Jersey are a plus.
Position Summary: We are seeking experienced and compassionate Registered Nurses (RNs) to join our Home Visit Concierge Service. This per diem/agency role is ideal for nurses who thrive in fast-paced, dynamic environments and can pivot quickly as patient needs and operations evolve. The RN will perform initial assessments and reassessments in the patient’s home, delivering high-quality, person-centered care that ensures safety, wellness, and care continuity.
Key Responsibilities:
- Conduct comprehensive initial nursing assessments in accordance with agency policies, state/federal regulations, and payer requirements.
- Perform reassessments on a routine schedule or when changes in patient condition are observed or reported.
- Accurately document clinical findings, assessments, interventions, and follow-ups using the agency’s EMR system.
- All documentation must be completed and submitted by the end of the day (EOD) of each visit—timely, complete documentation is a non-negotiable requirement.
- Collaborate with interdisciplinary teams to ensure coordinated care plans and address patient needs in real time.
- Identify and report significant findings or safety concerns, and initiate appropriate follow-up actions. Educate patients and families on condition management, medications, and health maintenance strategies.
- Maintain professional etiquette and cultural sensitivity during home visits.
- Participate in onboarding, including 4 hours of documentation training and 1 day of shadowing with an experienced RN.
Required Qualifications:
- Ability to work independently in patient homes and exercise sound clinical judgment .Strong organizational and communication skills
- BLS Certification (Required)
- Comfort with mobile technology and EMR systems
