PA Lifesharing, LLC is seeking an experienced and highly organized Quality Assurance & Incident Management Manager to oversee all aspects of Incident Management (IM), Quality Management (QM), compliance, and regulatory oversight within our MH/IDD programs. This position plays a critical leadership role in ensuring the health, safety, rights, and well-being of individuals receiving services while maintaining compliance with all applicable Office of Developmental Programs (ODP) regulations, Incident Management requirements, and quality improvement standards.
Compensation is negotiable and will be based on the candidate's qualifications, experience, certifications, knowledge of ODP regulations, Incident Management, Quality Management, and overall experience within the MH/IDD field.
The ideal candidate will have substantial experience in the Intellectual and Developmental Disabilities (IDD) and/or Mental Health field, possess strong knowledge of ODP regulations, Incident Management processes, and Quality Management principles, and have already completed Quality Management (QM) certification training. Bilingual English/Spanish candidates are strongly preferred.
Preferred Candidate Profile
- Current or previous experience working within the MH/IDD field.
- Extensive knowledge of Pennsylvania ODP regulations, Incident Management, and Quality Management systems.
- Quality Management (QM) Certification already obtained.
- Experience conducting investigations, incident reviews, corrective action planning, and compliance monitoring.
- Strong analytical skills with the ability to identify trends and develop quality improvement initiatives.
- Bilingual English/Spanish preferred to support diverse individuals, families, and stakeholders.
- Excellent written communication and report-writing skills.
- Ability to maintain confidentiality and handle sensitive information professionally.
Essential Responsibilities
- Incident Management (Primary Responsibility)
- Oversee all aspects of the agency's Incident Management Program.
- Review, investigate, track, and manage reportable incidents in accordance with ODP regulations and agency policies.
- Ensure incidents are entered, monitored, and finalized within required timeframes.
- Conduct incident investigations and root cause analyses.
- Develop and monitor corrective action plans.
- Identify incident trends and recommend preventive measures to reduce risk.
- Provide guidance and support to staff regarding incident reporting requirements.
- Ensure the health, safety, rights, and welfare of individuals receiving services are protected.
- Attend various meetings
- Quality Assurance & Compliance(assist)
- Assist with implementing and maintaining the agency's Quality Management Plan.
- Assist with monitoring compliance with ODP regulations, 6100 regulations, waiver requirements, and agency policies.
- Assist with conducting internal audits, record reviews, and quality assurance assessments.
- Assist with analyzing quality indicators and performance measures.
- Assist with preparing quarterly and annual quality management reports.
- Assist with tracking corrective actions and ensure timely resolution of compliance concerns.
- Assist with licensing reviews, audits, monitoring visits, and regulatory inspections.Data Analysis & Reporting
- Collect, analyze, and maintain quality and incident-related data.
- Identify trends, patterns, and areas for improvement.
- Prepare reports for leadership, regulatory agencies, and stakeholders.
- Develop recommendations to improve service quality and regulatory compliance.
Training & Support
- Train staff on Incident Management procedures, quality initiatives, and regulatory requirements.
- Provide ongoing coaching and technical assistance to direct support professionals.
- Promote a culture of quality improvement, accountability, and person-centered practices.
Qualifications
- Bachelor’s degree in human services, Social Work, Psychology, Healthcare Administration, Nursing, or a related field preferred.
- Minimum of 3 years of experience in MH/IDD services, Quality Management, Incident Management, Compliance, or Program Oversight.
- Quality Management (QM) Certification preferred.
- Knowledge of Pennsylvania ODP regulations, Incident Management Bulletin requirements, and 6100 regulations required.
- Bilingual English/Spanish strongly preferred.
- Proficiency with Microsoft Office and electronic documentation systems.
- Valid driver's license and ability to travel as needed.
