What You'll Do
- Provide mobile primary care to geriatric patients in assisted living, memory care, and adult family homes
- Diagnose, treat, and manage acute and chronic medical conditions
- Manage chronic diseases including CHF, COPD, diabetes, hypertension, dementia, and more
- Collaborate with caregivers, families, specialists, hospice, and home health partners
- Utilize our robust EMR and clinical support team to deliver exceptional patient care
What We're Looking For
✔ Active Washington Nurse Practitioner License
✔ DEA License (or ability to obtain within 90 days)
✔ Current BLS/CPR Certification
✔ Passion for caring for older adults
✔ Excellent communication and relationship-building skills
✔ Reliable transportation and a home office for occasional telemedicine visits
